Analysis of Age- and Gender-related Differences in the Patellofemoral Morphology in Asymptomatic Individuals: Magnetic Resonance Imaging Study

Objective: The aim of this study was to determine age- and gender-related differences in the patellofemoral morphology in asymptomatic individuals by utilizing magnetic resonance images.

Materials and Methods: Between 2018 September – 2018 December, 92 volunteer individuals without knee pain were evaluated prospectively. Cases were classified into 3 groups according to their age: Group I: 18–39 years (n=34 cases); Group II: 40–59 years (n=32 cases) and Group III: 60 years and older (n=26 cases). In all individuals, patellar morphology (the ratio between medial and lateral facet lengths and inter-facet angle), trochlear geometry (femoral sulcus angle, trochlear depth, trochlear facet asymmetry, lateral inclination angle, and patellar translation) and patellar height (Insall–Salvati ratio) showing static patellofemoral structure and tibial tuberosity-trochlear groove distance (TT–TG) showing dynamic patellofemoral anatomy were measured.

Results: There were significant differences in the trochlear sulcus angle, trohlear depth, trochlear facet asymmetry, patellar translation, Insall Salvati ratio, patellar medial / lateral facet ratio and TT–TG distance between the both genders detected (P value of 0.045, 0.001, 0.001, 0.006, 0.005, 0.001 and 0.032 respectively, independent sample t-test). As the age of the individuals increased, positive differences were found in trohlear sulcus angle (P=0.003) and Insall Salvati ratio (P=0.017) and a negative differences was found in trochlear depth measurement (P=0.026).

Conclusions: There are age- and gender-related significant differences in the patellofemoral morphology by asymptomatic individuals. Women and older individuals have a knee structure which is more prone to develop patellofemoral malalignament.
