Our Results on Microsurgical Testicular Sperm Extraction in Nonobstructive Azoospermic Patients and Evaluation of the Affecting Parameters

Objective: To evaluate the factors affecting the success in patients who underwent microsurgical testicular sperm extraction (micro-TESE) for nonobstructive azoospermia (NOA).

Materials and Methods: Age, duration of infertility, operation history, serum follicle stimulating hormone (FSH), total testosterone levels in 81 patients and the effect of these parameters on the success in sperm retrieval were evaluated retrospectively.

Results: Micro-TESE positivity was observed in 37 (45.7%) and TESE negativity was detected in 44 (54.3%) patients. Patients with a mean age of 33.8±6.6 (22-60) were divided into 2 groups according to their age. By comparing of these two groups in terms of sperm positivity in micro-TESE, no significant difference was found. (P=0.778). The duration of infertility were between 1-21 years. Eight patients (9.8%) had genetic anomalies. TESE positivity was found to be significantly lower in Klinefelter patients (P=0.029) The mean FSH was found to be significantly lower in micro-TESE positive patients (P=0.04) and there was no statistically significant difference in testosterone levels (P=0.109). While testicular size was observed having no effect on sperm retrieval (P=0.3), the probability of finding sperm was found to be lower in those with maturation arrest, Sertoli cell only and fibrosis in histopathological evaluation compared to hypospermatogenesis.(respectively; 35%, 26%, 14.9%, 91%)

Conclusion: While there is a statistically significant relationship between sperm retrieval success and accompanying genetic disease, serum FSH levels and testicular histopathology in micro-TESE, the relationship betweenother parameters and the effect of spouse age on pregnancies with sperm obtained aren't clear.
