Distinct Mark on the Face Caused By Craniotomy in Forensic Medicine: A Case Report

In Turkish Penal Code, the concepts of distinct mark on the face and permanent change on the face have been specifically addressed. In this study, formation of a distinct mark on the face due to surgical treatment as a result of injury caused by trauma was found interesting and the aim of the study was to discuss this incident with the literature and to raise awareness in preparing a forensic report.

In the brain CT examination performed in the hospital where the case was taken after being injured with a blow of stick to the head, right temporoparietal linear bone fracture and epidural hematoma were observed; thus, the case was taken to surgery and frontotemporoparietal craniotomy was performed. When the case was evaluated in terms of distinct mark on the face after the scar healing process was completed, it was found that a 3x3x2cm depressed skin deformity as a result of craniotomy in the hairless area in right frontal region caused distinct mark on the face.

In closed fractures that occur in the facial area due to trauma, by presence of surgical scar tissue or deformity appearance within the border of the face after the surgical intervention performed during the treatment process, it is evaluated in terms of “distinct mark on the face” since this appearance has a cause and effect relationship with the injury. Although our case did not have any scar tissue or scar that can be evaluated as distinct mark on the face as a result of injury caused by assault, because the deformity caused by craniotomy on the right frontoparietotemporal within the border of the face as a result of surgery had a causal relationship with the incident, it was evaluated as distinct mark on the face. It is recommended for physicians who prepare the forensic report to make an evaluation by considering this issue.
