MONITORIZATION OF NEUROMUSCULAR BLOCK AND ANESTHESIA DEPTH OF INHALATION AND PROPOFOL ANESTHESIA WITH THE COMPARISON OF HEMODYNAMIC AND NEUROENDOCRINE RESPONSES

In our study we aim to get effectiveness of train of four (TOF) and bispectral index (BIS) by evaluation the data of TOF and BIS via hemodynamic and neuroendocrine responses.

Eighty cases premedicated as ASAI-II and randomly grouped into sevoflurane, isoflurane, desflurane and propofol groups. Mean arterial pressure (MAP), heart rate (HR), BIS and TOF values recorded preoperatively, during entubation and after entubation with 10 minutes interval. The blood samples collected preoperatively, 30 minutes after surgery, at postoperative 24th hours and plasma glucose, ACTH, growth hormone, prolactin, insulin and cortisole is studied. Kruskal Wallis, chi-square, oneway ANOVA spearman correlation tests are used for datas analysis.

There were no demographic statistical differences between groups. In the sevoflurane group parallel correlation obtained between BIS values and MAP, HR, however that correlation not obtained in isoflurane and desflurane groups. There were statistically significant relationship between endocrine parameters and BIS values in sevoflurane group. No correlation seen between TOF datas and MAP, HR in three groups. Correlations were obtained between TOF datas and endocrine parameters in three groups. In the propofol group there were no correlation between BIS, TOF values and MAP, HR. The relation is obtained between all endocrine parameters and BIS, TOF datas.

As a conclusion both TOF and BIS values will not be certain guides for hemodynamic responses, however, we think that they give safe information about endocrine responses.
