Role of Frozen Sectoin Evaluation In The Surgical Management Of Multinodular Goiters

Frozen section evalutation is widely used in thyroid for the surgical management. Although the frozen section results are benign, malignancy is still a probability. The doubth of malignancy sometimes determine the extend of surgery. In this study we performed lobectomy to the lobe that frozen section was examined and evaluated, if the surgical choise was suitable with the histopathological results.

Four hundred seventy four patients were operated for multinodular goiter between December 1996 and November 2006. Hundred sixty seven patients with benign frozen section results were included to the study. Patients were divided into two groups: group 1 (total lobectomy) and group 2 (subtotal lobectomy). Groups were compared with the histopathological results and postoperative complications.

There 90 patiets with lobectomy and 77 patients with subtotal thyroidectomy. Three papillary cancers were reported in the lobectomy and subtotal thyroidectomy groups. Completition thyroidectomy was performed to these patients. Postoperative hemorrhage was developted in one patient. Temporary hipoparathyroidism was 6,6% in group 1 and 3% in group 2.

Frozen section evaluation has a small percent of misdiagnosis. This does not change with the use of total or subtotal lobectomy
