Factors Affecting the Quality of Life, Anxiety and Depression Levels in Ankylosing Spondylitis

Ankylosing spondylitis (AS), is a chronic inflammatory rheumatic disease predominantly involving the axial skeleton, has negative impact on quality of life. The aim of this study was to determine the factors effecting the health related quality of life and the levels of anxiety and depression in patients with AS. The study involved 60 AS patients, diagnosed according to the modified New York criteria and 60 healthy adult controls. The groups were comparable with respect to the age, gender, body mass index (BMI), marital status, level of education, job status, use of tobacco and alcohol. Participants completed questionnaires including disease activity (Bath Ankylosing Spondylitis Disease Activity Index, BASDAI), spinal motility (The Bath Ankylosing Spondylitis Metrology Index, BASMI), function (the Bath AS functional index, BASFI) and health-related quality of life (ASQoL). For evaluation of the patients and controls Beck depression scale, Beck anxiety scales were used. The level of education and job was the significant determinants of quality of life among AS patients. Additionally, female AS patients had significantly higher levels of VAS and BASDAI values. AS patients had significantly higher levels of depression and anxiety. The patients with higher disease activity had significantly poor quality of life, there was significant correlation between ASQoL and BASDAI, BASFI, VAS, BASMI and depression and anxiety scores. Results indicates that AS patients had a high level of depressive and anxiety symptoms than the controls and social and psychological support posses beneficial potential for AS patients.
