Radiographic Evaluation of Flexible Pes Planus in Children on Weight-Bearing and Nonweight-Bearing

Objective: Flexible pes planus is one of the commonly encountered deformities during childhood period. In flexible pes planus, medial longitudinal arc absence or collapse is observed when the weight is transferred to the foot. Nowadays, when assessing medial longitudinal arc, various angles, which are measured in radiologic examinations, are often considered. It is aimed to determine their relationships with each other when evaluating weight-bearing and nonweightbearing radiography of foot in some angles.

Materials and methods: A total of 27 children (15 girls and 12 boys) between the ages of 7 and 14 who applied to the outpatient department with complaints of flatfoot were assessed retrospectively. The weights and heights of children were recorded. Calcaneus pitch, talohorizontal, talo-first metatarsal and lateral talocalcaneal angles were evaluated on weight-bearing and nonweight-bearing lateral radiographies of foot. The Wilcoxon test was used for statistical analysis.

Results: In spite of nonweight-bearing radiographs, values of calcaneus pitch, talo-horizontal and lateral talocalcaneal angles were observed significantly lower in weight-bearing radiographs (P<0.001). There was no significant difference in talometatarsal angle values (P>0.05).

Conclusion: In both weight-bearing and nonweight-bearing radiographs, talometatarsal angle is a decisive measurement in diagnosis, but calcaneus pitch, talo-horizontal and lateral talocalcaneal angles are measurements which are considered to be helpful in the diagnosis only on weight-bearing radiographs.
